The collaboration aims to equip healthcare organizations with human-centered knowledge, skills, and tools necessary to create more inclusive, accessible, and culturally responsive patient experiences. By combining PX Academy’s expertise in Patient Experience best practices with Concordant Healthcare Solutions’ deep understanding of healthcare disparities and social determinants of health, the partnership will provide an engaging and comprehensive learning experience to drive meaningful and sustainable change.

Measuring and Reporting on Health Equity

  • Identifying relevant metrics and key performance indicators
  • Developing data-driven strategies to address equity gaps
  • Communicating progress and impact to stakeholders
  • Designing data collection, analysis, and reporting strategies to improve value-based patient experience scores
